AbstractThe time for providing outpatient medical record documents based on Permenkes No.129, 2008 is 10 minutes starting from the patient registering until patient's document provided at polyclinic. In a preliminary study conducted documents, there were 7 (70%) whose of provision was not following minimum service standards. If improved, it will have an impact quality and satisfaction. The type research used descriptive quantitative with cross-sectional approach. population number visits totaling 85,727 13 existing polyclinics. Determination sample size using Slovin formula obtained 100 documents. samples polyclinics proportionally. Methods data collection by observation interviews. results showed that average 19.94 minutes, established standards standard operating procedures. Factors affecting delay in 5M models found to be 2M had effect, namely human factors methods. Studi pendahuluan yang dilakukan terdapat waktu tidak sesuai standar pelayanan (SPM). Jika tersebut perbaikan, akan berdampak pada mutu dan kepuasan pasien. Jenis penelitian digunakan deskriptif kuantitatif dengan pendekatan cross sectional. Populasi jumlah kunjungan berjumlah 85.727 poliklinik ada. Penentuan besar sampel menggunakan rumus Slovin, didapatkan DRM. secara proporsional. Metode pengumpulan observasi wawancara. Hasil rata-rata 19,94 menit, belum SPM RS prosedur operasional (SPO) sudah ditetapkan. Faktor-faktor mempengaruhi keterlambatan model ditemukan M berpengaruh, yaitu faktor manusia metode. SDM bagian filing baru petugas (57,14%) mempunyai latar belakang pendidikan tiga hanya (28,57%) mendapatkan pelatihan dalam pengelolaan filling.
